Deathfire is really good with cassio, because it skyrockets our poke potential and we will build huge amounts of AP pretty naturally. The only other option really is Thunderlords, but I think piercing thoughts will do more for us than precision. The last big choices are wanderer vs. savagery and I like to have a little easier time last-hitting and cookies vs. assassin which really is going to be decided in champion select when we know our matchup and can decide if we need the reach to kill or the sustain.

Starting Items

    if we consider going for a second doran's ring we should take the seal instead

Core Items

    forget tear, this thing is what we need in the toplane, rush it!
    kill things until they are dead, twice
    AP = damage! and it kinda synergizes well with the passive the clock saves our ***

Situational Items

    well, those are all ok-ish

our items give us two essential stats: AP and health

since we don't have burst but more of sustained damage we need to stay alive a little longer, so health is really good
also we are in the toplane, this is not the midlane (obviously) so the distance between towers is much longer and we are not very mobile, so we want some surviveability
if we don't brainlessly spam our abilities the mana we get from RoA is far enough for our needs
the rylai's-liandry's combinaiton gives us three insanely good things: damage, lots of damge + utility to chase/keep in E range + utility to kite (we are not very mobile, remember?)

what i think about the situatonal items:
the "trade-offs" are:

RoA and/or Seraph: i think the stats we get out of RoA is "enough" in the sense that we still reach over 1000 Ap with an elixir and the above build so the extra health is worth more than the active throughout early and midgame, to be fair i wanna state that cassio can stack the tear decently fast, so going for a tear late-ish in the game is still possible if you desperately want to

Liandry's vs Voidstaff: this comparison is not entirely fair, because we also want the mask to synergize with ryali's BUT the flat magicpen we get out of lyandri's, sorcboots and runes is far enough to kill squishies and full-tanks or rather bulky champs tend to simultaniously stack life with their magic resistance (by spirit visage or banshees) so the passive of our mask makes up for the "lack" of "pure magic pen"

Ludens vs rylai's: both items have utility around movement, and the same ap stats, but rylai's gives life and boosts our liandry's, so this one is easy

abyssal vs Zhonyas: the "defensive choices", so is it armor or magic resistance? no thats not the question, zhonyas gives more ap, so more damage and a very nice active, the aura on abyssal is not that great since we don't want to get close anyways

as i said before, the above itemcomp suggestion relies on a lot of synergy within itself and gives us sustain that we need for the toplane because (lets be real) we are alone in the toplaneisland
(the most "replaceable" item of the above build is maybe zhonyas, so decide individually what you need to finish off your build, i think zhonyas is never wrong but maybe you find situations where another item is simply "better")
i don't really think the mejais-ring-thingy is not really worth

Lets talk a little about what we wanna do as Cassio

Number one priority is farming i guess, we always wanna be farming and go for kills only if our opponent gives us a very good opportunity. As we are a ranged champion our second priority is poking our opponent out of lane as hard as possible. The idea is placing Q under him whenever he wants to get a lasthit. By doing this we will also stack our passive decently fast. As Cassio is not very mobile we will very rarely gonna look for extended roams and keep farming to be the lategame beast we are born to be. All the behaviour for mid and lategame is highly influenced by how the early game played out and is also up to personal playstyle, team comb etc. so i'd rather talk about a couple of mentionable mechanics on Cassio:
- when we cast E on a poisoned target, the cooldown is reduced to basically nothing, this is our source of waveclear and damage on the enemy
- our W gives vision
-poisoning with Q lasts long enough for 3 Es
- when we lasthit a minion with E we get a good chunk of mana (and health if our passive is stacked high enough) back, so this gives us good lanesustain and mana management
- triggering R and then flashing will cause the ult to instantly go off after landing the flash, this technique is an awesome engage tool (pick the situations carefully)
- for Cassio focus in teamfights is a little different than most other champs, the rule for us is just to focus whoever is closest to us, as we are not mobile we can't be picky with targets and as our itembuild allows us to melt basically everyone we don't really care aswell
